Have you ever watched CSI? They had this one episode where some guy fell off the top of a Hotel building in Los Vegas- think about it, buildings in Los Vegas are huge. The only wounds he suffered that could be seen were bleeding from mass hemorrhaging of the brain, which means basically that his skull cracked. However, it couldn't be seen as an external injury.
